ci: shorten Claude review comments and raise the bar on re-reviews
The Claude review workflow posted long, padded inline comments, and every force push produced a fresh batch of new findings on code that had not changed. Both make authors stop reading the reviews. Add a comment style section to the prompt that limits each comment to the problem and the obvious fix, and truncate anything over 1000 characters in the post job. Track the number of completed reviews and the last reviewed head in a hidden state line in the tracking comment. Each successful review narrows the severities that may be posted: everything on the first review, no nits on the second, and only must-fix findings from the third on. The post job enforces the policy so it holds even if the model ignores the prompt, and reviewers are told to range-diff against the previous head and skip non-must-fix findings on unchanged code. Commenting "@claude review full" resets the count for that run.

## Review round
This pull request has already had ${{ needs.setup.outputs.review_count }} completed
review(s). New inline comments in this round may only use these severities:
${{ needs.setup.outputs.allowed_severities }}. The workflow discards any comment with
another severity, so do not produce such findings, do not send reviewers looking for
them, and do not list them in the summary. Each completed review raises the bar: a
finding that was not worth raising as must-fix in an earlier round is not worth raising
now unless the code it concerns changed. When `previous_head` in
${{ github.workspace }}/claude-review-work/pr-context.json is set and that commit is
available locally, use git range-diff and git diff between it and the current head to
identify what changed since the last review, concentrate on verifying that earlier
findings were fixed and on code that changed since then, and do not raise new
non-must-fix findings on code that was already present unchanged in the previously
reviewed revision. When `previous_head` is null, nothing was pushed since the last
review or a full review was requested, so review the whole diff.
## Comment style
Inline comments are read by busy maintainers. Shorter is better: a comment that fits
in one or two sentences is far more likely to be acted on than a paragraph. Each
comment body must state the problem and, when the fix is obvious, the fix, and nothing
else. Do not explain why the problem matters, do not describe what the code does, do
not restate the diff, do not quote the code being commented on, do not add headings,
bullet lists, bold text, greetings, or closing remarks. Prefer a GitHub suggestion
block over prose when a small replacement fixes the issue. A trivial bug gets a
one-line comment such as "Off by one: this should be `<=`." Aim for under 300
characters. Never exceed 1000 characters; the workflow truncates longer comments.
